SUPPORTING Indigenous-led health transformation

Birchwood Consulting led the development of the 2025-2030 Health Transformation Strategic Plan for Matawa Health Cooperative, serving nine First Nations communities in northern Ontario. Through extensive engagement and strategic facilitation, the plan defined five priorities and a phased implementation roadmap aligned with federal Health Transformation policy.

Advancing coordinated care through evaluation

Birchwood Consulting led a midterm evaluation of a provincial initiative focused on improving coordinated care for individuals transitioning between correctional facilities and community-based health services. Through data analysis and stakeholder engagement, the evaluation identified opportunities to strengthen system integration and enhance real-world service delivery.

Strengthening integrated team-based primary care in Ontario

Birchwood Consulting developed an implementation toolkit for the Association of Family Health Teams of Ontario to support interprofessional collaboration. The toolkit provided practical guidance and structured tools to help teams operationalize provincial policy directives and improve service delivery.
